ChronWell brings new lines of revenue, backed by Medicare-approved programs focused on targeted populations, to medical practices at little-to-no risk by offering remote, technology-enabled products and services such as Chronic Care Management (CCM), Principal Care Management (PCM) and Remote Patient Monitoring (RPM) services.

The Chronwell program has very little costs to the practice. We partner with your organization and charge a set fee for the approved CMS codes. The practice is provided a billing file that is sent to Medicare and ChronWell will invoice the practice for the contracted fees associated with each bill.

ChronWell’s Care Managers will reach out to eligible patients to explain CCM/PCM/RPM services and their benefits. If patients express interest, Care Managers will enroll them in the relevant programs.

Upon enrollment, an individualized care plan is developed for each patient, including goals, barriers, action plans and expected outcomes. Then, on a monthly basis, the Care Managers will provide a combination of the following services to the patient:

• HEALTH COACHING (DIET, EXERCISE)
• GOAL SETTING
• MOTIVATION AND EMPOWERMENT TO SELF-MANAGE AND ACHIEVE BEST OUTCOMES
• EDUCATION ABOUT THE CONDITIONS AND CARE PLAN ACTION STEPS
• MEDICATION ADHERENCE TRACKING
• LIFESTYLE CHANGES AND HEALTHY HABIT DEVELOPMENT
• COACHING ON MANAGING STRESS AND ANXIETY
• CONNECTING TO RELEVANT COMMUNITY RESOURCES
• ASSISTING AND ADVOCATING FOR THE PATIENT IN VARIOUS CARE SETTINGS
• SERVING AS THE LIAISON BETWEEN THE PATIENT AND PRACTICE OR OTHER CARE TEAM MEMBERS

Any outreach to the patient is white labeled under the physician and/or the practice to maintain the patient’s engagement and to serve as the extension of the medical practice. Our goal is to enable the physician to provide care beyond the face-to-face encounters and for the patient to perceive a continuity of care while feeling cared for and supported.

ChronWell CCM services are delivered by Medical Assistants (MA) and Licensed Practical Nurses (LPN), under the oversight and supervision of Registered Nurses (RN), who are also involved in the care delivery. 

LIVErHEALTHY™ is a Digital Health program designed to help practices manage NAFLD/NASH.  LIVErHEALTHY™ offers providers a new line of revenue with no up-front cost based on new programs approved by Medicare and private payors focused on targeted populations.

LIVErHEALTHY™ has no up-front cost to the practice and little to no out-of-pocket cost to covered patients. We partner with your organization and charge a set fee for approved reimbursement codes. The practice is provided a billing file that is sent to payors to streamline billing.  There is also a user-pay option for patients not covered by their insurance.

• Behavioral therapy: LIVErHEALTHY™ deliver timely interventions to keep patients engaged in the program and motivated to develop a healthier lifestyle.
• Diet: LIVErHEALTHY™ builds a personalized medical diet with recipes and prepared meals.
• Exercise and remote monitoring: LIVErHEALTHY™ includes a personalized exercise plan and remote monitoring using the latest wearable technology.

LIVErHEALTHY™ is delivered via the Livvy™ App in combination with ChronWell’s licensed care managers as an extension to the medical practice.
