Telephonic Care Manager

Sunrise, Florida

The Care Manager is an integral part of the overall patient's journey.  This role provides ongoing activities and follow-up to the patient population served beyond the walls of the physician's office. The care manager serves as point of contact for patients, and most importantly, is a patient advocate who ensures patients receive appropriate care coordination, support and education based on patient needs and provider recommendations. The care manager strives to assist patients achieve their goals that improve their self-management skills and overall quality of life.

This positions is 100% telephonic from our office location in Sunrise, Florida. Currently remote from home until further notice.
